1.    This is an example of a worksheet that must be completed prior to instruction. Its purpose is to identify students’ areas of strength and weakness so that we can pinpoint instruction and provide intervention so that all students can succeed.

2.    It demonstrates my knowledge of utilizing data as a tool to inform instruction. It also demonstrates my leadership skills as my form was used as a model for other teachers in the building. Additionally, I assisted colleagues in putting together, analyzing and understanding their own students’ data.

3.    Standards addressed in this appendix include: CCCT 2.7, 3.2, 3.5, 3.6. It also demonstrates my knowledge and understanding of CTTC Standards 1A, 2D, 3A, 3C, 3D. NETS Standards addressed include: 5A, 5B, 5C, 5D.
